Care Connections In Warrensburg Wins National n4a Award
Care Connections for Aging Services is among the recipients of a 2012 n4a Again Achievement Award for its “Shared Care” program.
Shared Care is a collaboration of three partners who wanted to improve community health and wellness through integrated quality health, wellness, recreation, and care coordination provided at one location for all stages of life. The Shared Care concept consists of a four-step process to help the participant’s lead healthier, happier lives. The care managers educate participants about health improvement options; network partners and care managers work together with participants to help them achieve self management goals established in their individualized care plan; care managers work with participants to ensure they have confidence in their ability to achieve their goals; and care managers monitor participant progress with the participant and agree upon modification to the plan as needed.
The prestigious award will be presented during the n4a Annual Conference which will be held this year in Denver, Colorado from July 7 through 11.
